There's several kinds of 'maid' distinction, this particular one was probably based on governess, which essentially serves as live-in etiquette tutor for higher ups' kids.

File this under reasons I prefer chopsticks over western style cutlery, most of the time anyways.

Basic answer for this one - always use the cutlery furthest away from the plate (which is then removed at the end of the course).

They stopped using this though (likely because it wasn't as effective as initially thought), instead opting for the more graphic depiction of bad effects of smoking, like a lung cancer (can't show them here for obvious reason).
