I think it's supposed to be a guy pounding rinami, but the artist forgot to make the dick transparent. And that's why 2 balls.

It's probably an attempt to make an "afterimage" effect to portray thrusting, but it's not transparent enough so it looks weird. post #10891979 as an example of what it usually looks like
