third eye has always been in a rough spot, because 44k out of 56k posts are of floating eyes on strings belonging to two Touhou characters. This has caused a number of issues in the past, such as not being able to imply it to extra eyes, or some tags about eyes not being able to imply tags about the face because of them - see forum #343089 for example.
Furthermore, another 6k posts are of Reiuji Utsuho. I would argue her thing doesn't even qualify as an extra eye most of the time, but that's an additional problem for another time. Maybe we should make eye-shaped jewel or something.
Another problem, also to be tackled at another time, is that disembodied eye should be split into disembodied eyeballs (post #10619532) and disembodied non-eyeball eyes (post #11042176), though we can keep them under the same umbrella tag if we want. We also have floating_eyes.
There's finally an argument to be made that third eye is the normal case, and we instead need a tag for more than three extra eyes, which are not easily findable at the moment. Maybe 4+ eyes.
Putting aside those problems, the komeiji sisters' eyes look nothing like the rest of third eye, nor do they look like what we have under disembodied eye, and I don't think they should be lumped in either tag or cause all the problems that they do, so I suggest moving them to an additional new tag, specifically for disembodied eyes that are still connected to the body. I'm sure we have more examples from original or obscure copyrights, but those are also unsearchable because we don't have a tag for "disembodied eyes still connected to the body in some form".
Feel free to suggest a better name, this is just to get the discussion going.
