Additionally, some kind of guidance on 'thresholds' of tagging would be nice: 'male_with_breasts -breast_padding -breasts' has 107 posts, and I feel the majority of these should either have one of the excluded tags or not have the topic focus tag.

I would have been quite satisfied to just use male with breasts for hong iltak or any other character with gynecomastia, but if it can only be used for male characters with womens' breasts ("impossible for real men"), then it would appear that my hands are tied. So I just decided to use the canonically-accurate term, which was already being used as a tag for a number of other posts, most I believe to have been inaccurate, and the remaining two (post #7700143, post #9247703) I didn't feel confident making a determination for.
For that reason, I would support an aliasing of the tag into male with breasts and a relaxing of the rule to include gynecomastia, as it is, technically, breast tissue. As I've already written into the wiki page, it is enough of a trouble that the various tags are used interchangeably on Pixiv, so I see no reason to keep the definition so strict when the name of the tag itself is so vague.
This reminded me how in topic #22122 (and mentioned later in topic #24031), manboobs was considered to be the gynecomastia tag, something rbc seems to disagree with as they labeled manboobs as adipomastia in a recent wiki edit.
