I agree that "I'm so fucked up" is the better, more well known name. Is it perhaps so much more well known that it should become the tag name? I have never once heard "Shinji's fapping hand" and it seems like it was just pulled out of thin air by the user who made that tag (now banned for making bad wiki edits for things he knew nothing about, if that carries any weight). "fapping" is a pretty dated way of talking about it too, and was when the tag was dubbed even, in 2024.

I couldn't find an "official" name for it on Know Your Meme or anywhere else, but I agree and would prefer this version. I have literally never heard this called "Shinji's fapping hand." And I would not think to search it since the scene is so synonymous with the quote.
Diet Soda is right searching the term only brings up Evangelion memes and song lyrics. If not a rename I'd like it at least as an utility alias.

This sounds like a made up problem. By this logic we should start renaming every one of the hundreds of other meme tags that aren't hyper-specific descriptions of the origin image, because in 20 years' time some other trend might pop up that involves the words imagine or always has been or special feeling. But this doesn't really happen, and I can't imagine what kind of impact this hypothetical rival meme would need to have to be confused with one of the most iconic lines from one of the most iconic anime of all time on an anime-focused site.

End_of_evangelion_hospital_scene would be a descriptive neutral term and matches tags like chainsaw_man's_handjob_scene, though I prefer i'm_so_fucked_up_(meme) since that is, again, what people call it.

The KYM name is The End Of Evangelion Hospital Scene but that name sucks
"I'm so fucked up" is definitely the most recognizable, and what you would search for on Google Images to find examples of this meme
