I'm out of ideas, but now that I think about it, there's no tag for situations where characters realize a gender swap has occurred either. Many posts either don't mention it at all or treat the situation as an AU where the character has always been the other gender

This also seems to be the case for transformation arts, some of them doesn't actually transform their clothes as well, making the genderswapped characters confused what is going on with them.

If the intent is to cover characters like Mordred (Fate), I think that would weigh against using "genderswap" in the tag name, because we don't typically tag the Fate "actually this historical/mythical person was always female" characters as genderswap, not least because they would swamp the tag to uselessness.

Best I can come up with gender_denial? Although that might be too close to gender dysphoria.

I had put genderswap clueless as well in my reply above since unaware is aliased to clueless, so I don’t know if you want to alias this too for consistency or…

Also, about genderswap denial. Maybe post #10903238, post #10875996 and similar could fit this concept? The current definition of denial doesn’t really fit those posts, in case of a potential genderswap denial search.

This post, however, seems to me that only one of these tags fits, but I don't know which one. On the one hand, Frieren notices the changes in her body, but she talks about them very descriptively, as if she didn't realize that they meant she had been transformed into a man.

Maybe, though I'm not too sold on this. Also it's kind of a counterpart to the genderswap aware proposed above, so they would mismatch as tags.

I don't think either "unaware" or "clueless" fit the posts that are in that tag right now. In most of those it isn't that the characters don't know about the genderswap, it's just that a character is being referred to as the original gender or claiming to be the original gender. Almost all of them I would actually call genderswap aware if we created that tag, because the characters clearly know about the genderswap:
