There really isn't one that I know of. I would say check out other artist wikis (Artist wiki search). The most recent substantive discussion on artist wikis that I can think of is topic #16716. One thing to note from that discussion is that including artist URLs in the wiki itself is now discouraged, and I've been slowly and surely removing them from wikis as I encounter them.
The following is the format that I use for sections when and only when they're needed. I haven't been creating the artist intros myself, I just take what is currently there and move stuff around as needed.
Artist info, such as nationality, preferences, and circle information.
- 1. Original characters (h5 header)
- Characters that do not appear in any of an artist's officially named works
- 2. Works (h5 header)
- Copyrights which an artist is the main or partial contributor to
- 3. Prior names (h5 header)
- Previous names that the artist has been referred to by on Danbooru
- 4. See also (h4 header)
- Would contain all important and related wikis/pools/forums on Danbooru
- 5. External links (h4 header)
- Would contain all external links
- Any external links that are needed in the prior sections should go here as references instead
Here is a list of recent artist entries that I have edited, which somewhat demonstrate the above.
1. Wiki pages not required. Create wiki if you have more info about artist.

2. Not duplicate artist page info (URLs, nickname).

Write basic info:
Names in kanji
Names in kana
Site name
Circle name
Pixiv nickname (find under pixiv avatar)
Pixiv username (find in the pixiv stacc feed URL)
Twitter name
But not need duplicate name and nickname from Artist page.
3. You can write short biography (when aviable and need) or use link to biography/artist description page. See howto:wiki.
Be concise. Provide only the basic information that someone unfamiliar with the subject needs to know. As a rule of thumb, avoid going beyond one to two short paragraphs. If possible, especially regarding characters and copyrights, provide a link to an outside website with further information for the benefit of those interested.
Not required, but can be useful for history.
