There are a number of hints that Saint Trina, an enigmatic figure named and referenced in several sleep and dream related items, was a disguise used by Miquella when travelling afar.
Miquella was spirited away by Mohg prior to the start of the game for his own nefarious purposes. By the time the Tarnished sees him, he has been encased in a cocoon where he has begun changing.
