From the looks of it, she's thinking that the Comicon cosplayers has aliens in disguise based on their endowments.

Alien-chan likely thinks that due to the size of her boobs, Kouhai-chan might be a some kind of wanted criminal hiding amongst the Comiket cosplayers (cue Joushi-chan running by in the background lol).

When she asks our MC, the image in his mind is *fake". At first, my thought was she thinks he is referring to her own. At which point, her response was to reaf-"firm" that hers are indeed, real.

Seeing the translated title, my thoughts now are that she's not convinced by his answer, and is using her special negotiation to milk a confession out of him.

Which begs the notion if maybe she's an alien detective...🤔
