So what does it say? Can't identify what alphabet its written in.
Though I do find it funny that Patchouli's name is just transliterated to Runic with some additional letters.

Let us just say that it was not meant to be translated, but Fumitsuki found the safest way to write the name without taking any liberties of actually giving her a name. The result is pretty boring, but I wanted to try and decipher it since it was not in Demonic or Infernal or other alphabets I knew and I could see some of the letters being used twice, which clued me in that it was not just cool looking scribbles and on closer inspection they are only fancy-fied old German style alphabet letters. Edit: Mirror each letter, but read them as they are already written. Makes it easier

The name is two words, each of 9 letters, where 3 of the letters are used twice, marked with XYZ.
ABCDEXFYG HIXJYZKLZ

I will not spoil it if you want it to remain a mystery or you want to solve it yourselves, but here is a hint for the letters used twice:

Is the upper name, Koakuma's true name, supposed to be just P-------- K--------, but written in a different font, then?

... Maybe I'm horribly wrong, but doesn't that line up with "Patchouli Knowledge"?

Dang, that's some deep lore. Didn't think the name was legible when I was first running through it, though I did notice that the first part of her name looked awfully familiar after Patchouli grabbed it.
