Well, they could either be fast at drawing, or have a "template" already set up, including a background and basic body, where they just need to slap a hairstyle and some recognizable facial details on it so that they're ready to claim the "first" lewd art.

This artist may made a lot of ready-make layout then, because Ark Royal's boobs size is correctly drawn instead of big boobs like other standard carrier does.

Note that she's face-down, and her arm covers nearly all of her breasts, so you only see that little triangle between her pits. You could pass that off as "the right size" for any size reasonably large... and for small, just not draw that little triangle of breast. (In fact, she looks larger her and especially in the other image he drew of her than in her official art, so he may just have had a large-sized breast image ready-made.)

Alternately, just leave the breasts undrawn so that you can quickly sketch it in like with the hairstyle.
