I actually think the artist meant to draw a Chiappa Rhino, but messed up the cocking handle.

The hammer is a fake hammer only for cocking for single action. Kind of like how the chiappa rihno revolver mechanism works. The real hammer is upside down.
If ya wanna get super angry about impossible guns, Vash the Stampede's gun in the anime has the hammer on top as standard while the barrel is aligned with the bottom of the cylinder.

Ah got it. I haven't watched all of trigun myself. The only discrepancy I can think of though is that cocking hammer moves when he fires in double action, but I think other inverted revolver designs do feature that kind of mechanism too, such as the Mateba which is in GitS.

Well.. I stand corrected and in awe. Not to mention the detail in that video to show the true mechanics of such a tremendously unique kind of system makes me thirst for more knowledge.
