The scan is kind of bad actually; I'm uploading in hope that someone can clean this up or locate a cleaner scan.

Reverted "error" tag as there is no error on this image (besides ugly scan). If you are having problem viewing the image, be sure you have sufficient memory to load a 3720 pixel by 10000 pixel image.
