Simple, because many of these fictional games either rely on futuristic consoles or are basically the same as currently existing MMOs, only with different player interactions because anime antics and stylized/exaggerated visuals as per rule of cool/drama.

Because gamers don't actually want most of that stuff.
Star Wars Galaxies actually had a good chunk of those roleplaying subclasses (before the NGE update stripped it all out). The vast majority of players never touched it, even though it was revealed (much later) to be a requirement to unlock the Jedi class.

That may be so in .hack, ID_entity, SAO, etc., but in Log Horizon it was shown that Elder Tale is played using a set up that we have right now (a PC with mouse and keyboard).

There's a lot of problems with Galaxies, and that was the least of them.
It all comes down to interest, and that if Elder Tale does exist, it would be indistinguishable from the multitude of existing fantasy MMOs out there already. That itself is not a problem, but no matter how immersive the game is or how intriguing the "story", publishers rely on a "gimmick" that makes the game sell. And having a "prostitute" profession isn't exactly a good selling point.
