That's because she's a Menreiki, a mask youkai based on masks that Prince Shōtoku made.

Also, Miko's ending in HM: Miko recreates Kokoro's missing "mask of hope", and it looks like Miko's face.

(that and the Touhou fandom has more ships than most navies)

IIRC the masks in question were used by Hata no Kawakatsu, so she sort of is both.

Ah. I'm sorry. I wasn't clear enough. Prince Shōtoku comissioned Hata no Kawakatsu to make the masks and further had Kawakatsu perform with it, thereby officially making/acknowledging the art of Noh. Shōtoku is credited with making Noh come into being, but Kawakatsu should get the credit for making the actual mask.
