Both hand grab and grabbing another's hand refer to one-sided hand holding, so one should probably be turned into an alias of the other. I didn't create a BUR yet because I don't know which should be aliased to which. hand grab is used more often and has a wiki page, but grabbing another's hand follows the same grabbing_another's_* naming scheme used for other body parts.
The latter, it suffers from the same issue the rest of these tags have.
Browsing hand_grab solo I can easily find random bad results due to its ambiguity such as post #8100126 (own hands together) or post #8157915 ("grabbing with hands", lol).
There's also a bunch of holding another's wrist/holding another's wrists in there.
I just took another look at it and there are actually so many wrong cases that it might be worth deprecating hand grab instead and referring to grabbing another's hand, holding hands or holding another's wrist. If aliasing is better however, then it is probably worth it to bulk remove hand grab if a post is also tagged with holding hands or holding another's wrist. Both hand_grab holding_hands and hand_grab holding_another's_wrist have quite a lot of results.
Also, holding hand is currently aliased to hand grab, but that should probably be aliased to holding hands instead. I don't know if that should be included in the same BUR or in a separate one.

remove alias holding_hand -> hand_grab
create alias holding_hand -> holding_hands
mass update hand_grab holding_hands -> -hand_grab
mass update hand_grab grabbing_another's_wrist -> -hand_grab
create alias hand_grab -> grabbing_another's_hand
What I had in mind looks something like this. It moves holding_hand to be an alias of holding hands instead, it removes hand grab from posts where it was used to mean something other than grabbing another's hand and then it does the final alias to grabbing another's hand. I think this is the right order of operations, but I'm not entirely sure.
It's not clear to me what you want grabbing another's hand to be for. Are you talking about something like post #11138868 or post #11135647, where only one character is doing the grabbing, and it's not reciprocated by the other character's hand?
Yes exactly, grabbing another's hand would just be what hand grab currently is as stated on its wiki page:
Grabbing another's hand. Between two characters, where only one character is actively grabbing the other's hand.
