Anniversary servant implications. Her display name / traits / etc change to Olgamally's in the third ascension, hence the different implication level & qualifier, but also leaving a utility alias similarly to Arcueid Brunestud (MoonCancer). The reason the second ascension is implied to u-olga should be fairly self-explanatory, I hope, and in the first ascension you can still see her signature horns.

Same logic as forum #374039. Generally speaking if a servant popped up somewhere before FGO, they usually don't need a tag for whatever that default appearance was in their ascensions. Mary's second ascension tag is about 2/3 of her posts, while Daikokuten is much more blatant at about 90%.
