The first time in the manga it's in chapter #351 and the official data book and the volume #34 extras mention it, too.

Regardless of whether the last name is known or not, most people know the character by just the single name. I think adding on the last name might just cause confusion.

I don't think "the character's full name isn't commonly known" is a good reason not to tag characters comprehensively, in the standard fashion. Aliases and autocomplete easily take care of any difficulty someone might have finding the correct tag.

This is an old reference, but back in the day kasuga ayumu wasn't commonly known by that name and yet we tag her as such.

So long as it's not a spoiler, I don't mind aliasing single names to full names. I do draw the line at names that require outside sources to know, like interviews or databooks; the name should be mentioned in-universe and readily apparent to people that only view the anime/manga/light novel/etc.
