Judging by the dialog, she's eating him slowly so that she can keep him alive to have sex with.

The dialogue does not suggest that she is the reason for his injuries.

The images do. She's attaching to his stumps before giving him head. Either he's kept alive for the sex or the sex is some sort of attempt to compensate for what she's taking from him.

In the end, it looks to me like he dies, and she calls him a coward for not sticking with her.
