I don't think any of this belongs in a translator's note, but the furigana for the kanji in the last line (永久機関/perpetual mechanism） say "sex machina", as the title suggests. I tried to write within the Danbooru translation guidelines, but the result was ugly. In those five lines, there's one—arguably two—actual verbs. If someone has a conniption fit I can rewrite it with no pronouns and 1.5 verbs.
I'd say "yes" to the question someone just asked me: "So am I to understand the piston action is actuated by his desire to penetrate (inspired by the smell of her feet) on the downstroke and her pushing off against his face for the upstroke?"
