Can someone explain why the rating was changed to explicit? Because according to howto:rate this image falls into questionable category.

This one is actually hard to rate, as the sexual content here is not "blatantly exposed", but it is meant to draw attention to itself.
IMO, this would be either questionable or explicit; it doesn't make much of a difference.
