Well it looks like she is in some vat of shit and there is something in her breast and either the smell or her breast are in pain. Well thats my guess

The "something" in her breasts are almost certainly Zouken's worms (I think that the other pictures prove this). And, I'd suspect that she's in pain (rather than just being the smell).
