On the other hand, did they have the thought of drawing Nimi here just because she's a nurse? I can really imagine the long privately held subject of Saba's health and the fact that Nimi loves Saba as a friend can make this image come off as in bad taste for people

This thought is from the perspective of the viewer and artist, not directly between the two vtubers.
As I said there would be two sides to this, the ones who see this as a supportive image for them and others who will interpret it as catastrophizing and feel like depicting it like this is fucked up. And you could argue then that it would have been better to depict the setting as a more happier nursing moment instead if you wanted to avoid some people to feel this
