1st answer: no, there's not. Endoscopic equipment doesn't need censoring and essentially works like video camera, with recording and playback function. Just a lot more specialized and pricy. Censoring would be something that's added in post-processing.
2nd and 3rd answer: Maybe someone...or the chief herself used the gear in uh, different way than it was intended and forgot to reset the settings.
