Maybe lot's of them just skip the text? I know I sometimes do that in some games, and then later regret having missed the characters nuances and memes.
I think the main thing is that Danbooru gets a lot of horny spam comments, so it's easy to gloss over the comments and think it's just another one of those. People that aren't Fate fans are in the community so they wouldn't know it's a reference, and it's from the new JP event so Fate fans that are only keeping up with the NA release wouldn't know the reference either. So there's a variety of reasons various people would think it's just another horny comment.
