To put it simply, this is the difference between a slap and a punch. With a punch, there's less surface, so the strength goes in the knuckles. With a slap, the strength is spread all over the palm. Same with a coat hanger, because it's thinner, all the strength is focused in a small part, so rather than leaving oval marks or handprints like a hairbrush or a hand would, a coat hanger would leave thin stripes, like a cane or a switch.

The thinner the stick, the more it will sting. It's not like the parent is actually try to hurt the kid, why would they use anything with mass? The thin stick will leave a very stinging pain and a red mark on the skin. It's not really hurt, but the stinging pain will last for quite a while.
