I think it's supposed to be Shenhe's left hand, at a glance it looks like Shenhe has both her arms behind her but it's Yelan's leg over Shenhe's left shoulder. We don't actually see Shenhe's left arm. It's very confusingly done, and the hand looks like it comes out of nowhere. That's the best explanation I have.

Seems an awkward angle, but it should be doable, though likely uncomfortable.

Yeah it looks possibly doable, but uncomfortable for really no good reason. It would look at least a bit more natural if the hand was positioned differently I think, so it can give a more natural indication of where the arm is.

Everything about the way this artwork is angled is confusing, but considering the character is a superhuman that can carry a multi-ton rock across half a country and battle a lesser god to a draw as a kid, I doubt bending her hand at an awkward angle is any issue.
