She also subjected another person to A LOT of pain by having them play the game. Simulated pain, but pain alright. Pain of being stabbed, pain of having your eyeballs gouged out, and other similar torture treatment for undisclosed period of time.

Kamui doesn't even count as her ex. He just told her he wasn't into girls with G-cups and she took it way too personally. Granted, Kamui was a jerk, but she's been holding a grudge for 10 years and he probably wouldn't even remember her.
And yeah, what Rathurue said is true: she's been subjecting people to this horrible torment for a while, all so she can spite Kamui. One has to wonder why the hell she doesn't just beat the game herself. She even tricks Hiro into continuing the game by claiming she'll sleep with him, but she's totally lying.
