So one, if it's still alive it'll puff up in her mouth at the least dislocating her jaw (idk the force a puffer-fish goes through in doing so) while also puncturing any soft tissue in her mouth.
Two, if it's dead I'm guessing the poisonous bit is still in the fugu given this is the suicide girl.

If you eat a fugu whole, of course the toxin would still be in.
