I think, no, it only applies to differences with canon. By rights the big tiddy version should be tagged as alternately sized, but since there are a lot of characters (mainly from Touhou) which are consistently depicted with non-canon breasts, there is apparently also a precedent of not using the tag on 'fanon consensus' breasts.
