The exchange Rumia had with Chen would imply that whatever it is, it has become a frequent event for her. So in one way or another it is something that both she and Reimu can walk or at least crawl away from.

Historically mikos did offer sexual services to patrons, so maybe Rumia is just filling in for Reimu's unofficial 'duties' so to speak in exchange for food or whatever.
