Sometimes they are professionals. Many professional manga artists and illustrators will self publish amateur side works free from the restrictions of their employers/publishing houses. What separates an amateur doujinshi from a professional manga is who published it, not who created it. Anything self published regardless of who the artist is, is considered amateur because it was not done for professional work or a publishing company. There are also many semi-pro artists who have regular full-time jobs and self publish doujinshi as a hobby, but have such a large following that they can sell their work for profit as a second job.
