Their outfits do have an awkward dissonance with the rest of the series. I think the reason is because they're not from gensokyo. They all have a funky outer space feel to them.

She's not a literal personification of the flag, but a Hell Fairy. However, I suspect her design was used for half-commentary, half-canon reasons. First, consider the "war" the Lunarians had with the first men on the moon, the one which caused Reisen to run away. Now here she is,
sent to the moon to invade with other fairies, bringing the threat of impurity and death to the Lunar Capital while dressed as the American flag. She's basically two things the Lunarians despise.
My personal thought is that maybe she found the flag and liked it so much she dressed herself in it, which is why the flag has disappeared in canon.

Clownpiece's objectives are to take over the Sea of Tranquility and make it a fairy playground so Junko can spread impurity easily, and to piss off Lunarians as much as possible. Now that the Sea is captured, she can just dress up in Murka' colors and go bother people like fairies do.

On one hand, you gotta admire how back then Nasa could buy stuff from local stores for a few bucks and use them in their space program. On the other, it's kind of sad to think something as iconic as the flag placed on the moon is probably gone forever due to it being of the same make as one you'd buy for a 4th of July weekend.
