It's a Kresta-II (Project 1134A Berkut-A). Major distinguishing features visible are the KT-106 quadruple missile launchers for the Metel anti-submarine missile abeam the bridge and MR-600 Voskhod "Top Sail" 3D air search radar atop the mainmast. (Kresta-I/Project 1134 Berkut used FKR-2 single launchers for the P-5 Pyatyorka anti-ship missile and MR-310 Angara-A "Head Net" in the same places.)

Both 1134s and 1134As had helicopter pads, although only the 1134As had them on raised platforms, which are visible in the picture.

I'm continually amazed by this artist's attention to detail - not only the ship, but the Mi-24 in the foreground, bort number Yellow 41, was an actual VVS Mi-24V wearing this exact paint scheme.
