A maid headdress and pads are enough to tag Sakuya but Koishi’s hat on a subconscious stone aren’t enough to tag her?! I’m intrigued by the character tag requirements.

People miss tagging things here and there.
Anyway why the artist picked a zombie Meiling is odd.

Let's see, if i go with fanon interpretation i think it is pretty much make sense. Meiling get stabbed all the time (most stab is in the head, no less) and somehow survived all the time, right? Guess she is either some sort of invulnerable youkai or, well, an undead.
Besides zombie Meiling, the only two I don't really understand are scallop Hatate and beansprout Tenshi.
The rest, I sort of get - bullet/suppository Udonge, yuki-onna Letty, snake-and-frog Sanae, ko-ishi Koishi, etc. (in Koishi's case, it's because her name is a homophone for "ko ishi" [lit. "little stone"]; Akyu likens her to a roadside pebble whose existence is only acknowledged while it's within your field of vision, and even then it's only noticed subconsciously)
For Hatate, it's a word play. ホタテ - scallop (hotate) For Tenshi, the artist added the tag まな板 (chopping board) to this.
Well, that does explain a bit, but it also raises a new question:
Why is Patchy depicted as a cutting board? She's almost never depicted as flat except for those art styles where everyone has the exact same body type (such as dora_e and walfas)
