Fate/Strange Fake character. The "False" Rider-class Servant of Kuruoka Tsubaki.
It is a formless black cloud said to be the embodiment of pestilence, and is given the True Name Pale Rider in homage of the Pale Horse, one of the Four Horsemen of the Apocalypse.
