In this costume, Hanako's ahoge is much larger compared to her standard costume. She wears simple set of pink bikini with blue outlines and strings, and an oversizedwhite shirt on top of it (likely belongs to Sensei), and a pair of black sandals. She holds a green garden hose that's connected to an old hand pump, as seen in her art and her EX-skill animation.
Due to the peculiar way Hanako's outfit was drawn (only her bikini top is visible under her white shirt), combined with the fact that her in-game chibi model does not appear to have a bikini bottom, this resulted in fan artists drawing Hanako in this specific outfit as being bottomless.
