    I think it has something to do with
    Madoka appearing right before Homura's Soul Gem turned completely black at the end of episode 11. If this is supposed to be after the series has ended, maybe she's trying to get Madoka back?
    Despite all that, I wouldn't be sure until it's at least fully translated.

    I'd say is more related to how Kami-Madoka appears before the magical girls who're about to turn into witches, to cleanse their soul gem and possibly take them to heaven or wherever she took Sayaka to. Homura just wants Madoka to pick her up
