Japanese love conspiracy stories like these. My problem is that Homura already knew all of this and I'm not sure why she only waited until now to reveal what is happening or why she doesn't give more information. Then again, she is only interested in Madoka so I doubt that she is being completely honest either.

You make a good point, gave me a bit of hope.
Anyway, when I think about it, I can't really remember a single fantasy or sci-fi themed show since 2000 that didn't revolve around conspiracies/judgement days/merging humanity into some sort of collective consciousness/amnesiacs/chosen ones/amnesiac chosen ones/future traveller friends/hidden powerlevels/fate, et cetera.
There will be LOTS of brownie points for Madoka from me if they manage to avoid cliches like that, if only to some degree (given the fact Homerun is pretty much confirmed for time traveller now).

That's... quite a big list you've got there. Of wildly different things. And it ends with "et cetera." And this is somehow supposed to be proof of a lack of variety?

The problem is that while these are a variety of different options, these are the basics of how a story is written now. They are all rather boring, at least in my opinion, not because of the concept themselves but how they are executed.
But really, how much can we expect from a magical girl show?
To be honest, I don't remember the last time I saw a dude die, then suddenly respawn beside the body in the way it happened here...at least, not outside an FPS, lol!

We can expect whatever we want from it. We can expect nothing interesting and simply stop watching, or we can anticipate such a masterpiece in anime that it forever remains a well-known series as One Piece, Nanoha, Code Geass, Hellsing, Tengen Toppa Gurren Lagaan, Cowboy Beebop, and others have become.
I expect the latter and continue watching without regrets...

Whatever you want to call it. My point was that you could potentially include absolutely everything in that list, so there's no point to it. Even if a series emerged that didn't happen to have any of the elements you mentioned, whatever defining elements it would happen to have could easily be incorporated into the list. I'm really not seeing any of it as evidence of any sort of new trend or tendency that writers have suddenly started resorting to in place of originality or imagination.

If I can narrow down the story to a series of words, that is not the indication of an original story. The problem isn't the variety of options, is that these options are predictable outcomes and cliches. Heart of Darkness, at its core, you could say is a pretty simple story but I could say, as well, that it is a deep story in that the basic elements far exceed the sum of their parts.
If Madoka does go the direction I think its going, it won't be original and everything up till that point will be neutralized. The greatest example, to me, of an ending completely that ruined the entire series in retrospect was Blue Gender.
