Generally, crossing fingers with the hand in your back means you're lying.

I have yet to understand the sense of this picture, though.

On the Touhou Clock thingy, there are some short animations where you can play Rock-Paper-Scissors with a character (often Reimu or Marisa); they use a sign much like that one to show what they cast (and it really does change even though the animations look the same).

So Sanae here has just gone and applied her [lack of] common sense to Rock-Paper-Scissors.
