In FF VII: Last Order (the animated short that came at the same time as Advent Children), it was shown (or retconned) that Sephiroth slashed her neck.

Yup, I think that's why she wears high-neck shirts. Her white top and black vest are much higher than her cowboy shirt.

Then again, this IS Final Fantasy we're talking about here; logic hasn't applied much to the characters and their waterproof cloths.
