Depends on different interpretations. Just like most opt to have Okuu's yatagarasu eye thing removable with her clothes too, instead of fused into her chest.
Besides them Komeji 3rd eyes look like clip ons anyways. (Satori's a hairclip on the head while Koishi's are clips on her boots.)

At least Utsuho's extra eye doesn't really do anything, (plus she also seems able to make the three legs/arm canon insubstantiate at will, so it may just be that she can make the physical changes from eating the sun god reverse at will), so that isn't such a big deal.

The eye that reads minds/hearts is an innate part of being a satori. It's why they were driven from the surface, and Koishi's willingness to actually close it, and turn her back on her fundamental nature is her primary characteristic.

If it's just a funny accessory, and they could just take them off, and avoided having been reviled and exiled, none of the plot makes any sense. Why be exiled when you could just take off your satori eye and blend in with everyone else?

Maybe Koishi's is removeable now because she closed it? Now that she can no longer read minds isn't it essentially a dead organ that serves no purpose? Almost like wisdom teeth except she wears it?
