If the character model is meant to be a boy, then I think it should be tagged as one. It's basically just an otoko no ko isn't it?

I don't think so. Otoko no ko boys are easily mistaken for girls unless someone points out their gender. This guy might be androgynous but doesn't necessarily pass for female to viewers.
