She wears black nail polish on her other hand. I think that's her finger, and that hand is on the chin to motivate the mouth for the "open up" command.

Oh yeah, that could be it too. The wrinkle in the sleeve behind it makes it look like a piece of fur sticking out.

Specifically, I think it's her thumb on the lips. If you're opening someone's mouth, you'd have your fingers under their jaw/chin and use your thumb to open their mouth.
