I'm gonna take a wild guess that the girl is his sister (they look quite alike) and that either an accident, illness, or both claimed her limbs, thus, with mounting medical bills and many other expenses, along with having very little to no help on the side, the boy is left with no choice but to work long hours to provide for them both, much to his sister's, who probably blames herself and is quite lonely, upset.
