Mahiro-chan in a setting where he is "a girl that sometimes becomes a boy" with just his sex going back, and Miyo-chan who has no common sense about medicine
Joke aside, I really wonder how Mahiro would cope if his sex kept changing constantly. The dude from Kampfer pretty much treats it as a transformation, the dude from that Only God Knows goes pretty deep and get really uncomfortable when he's in the girl's body, but for Mahiro, it's pretty much treated like... well, like waking up one day, and suddenly being unable to grow a beard, having to sit down to pee, suddenly finding Caim hotter than Manah, and so on. I once tried to ask a trans friend how it is, but in his case, he explained it was a feeling like "things aren't as they should be", and how he feels like he's possessing a body that isn't his.

The much older genderbend anime (the title is Ranma or something) sort of explains it if I remember.

Ranma 3/4? I though it was comical, but if it brings that up, might be worth reading.
That's right~Hey!Mahiron~Stop it, I'm a boy now!I'm fine with that!How... should I look at this?Ugh...I don't care! Even if she's a boy, Mahiro-chan is Mahiro-chan!That's not the problem here!
