^Ono said it's up to personal interpretation, the transexual/hermaphrodite story is not "canon" in any way. Back in the day there were problems with beating the shit out of women, so they said that as a quick and easy solution. Poison was made with every intention of being a woman right from the get go.

You never see her genitals in the game, so it shouldn't really matter.
