Anatomy-wise, the addition of that small heel will cause disturbance to the walking or running pattern of a horse (it's gonna break off pretty easily anyway). Unlike humans who will still put pressure on the heel area due to weight distribution, horses will never put any pressure on the 'heel' part of this kind of shoes because it's actually positioned on their ankles instead, obstructing the hooves pivoting motion during gallop or running.
