Once you type in the name/translation, resize it and place it in the proper place; place the cursor over the name/translation pop-up, click on it and press save.

Who is the large guy on the upper right-hand side? Is it Tokurou (the bone maniac) or Atsukuru (the newest kindergarten teacher)?

That's Usui himself - you can tell by the scarf and the hair flipped to one side, which is how he'd draw self-portraits. (Did he appear that way when he'd cameo in Shin-chan, as well? I'm not sure.)

Yeah, he appears like that in Shin-chan cameos as well.
