In canon Touhou media, is everyone on first-name terms? I thought Japanese people (ZUN is Japanese, and his work is primarily based on that language and culture) usually addressed each other by surnames with honorifics depending on relative social/familial status, but this comic (and many others on this site) has people using first names by default.

Yes, everyone is usually either on first-name yobisute basis, or they call each other by nicknames ("red-white", "black-white") or some variation of you (anta, anata, omae, etc.), or a title or race (you oni, human, tengu, ice fairy, etc.).
Things change when master-servant relationships are involved (Udonge calls Eirin "master" (as in the master-apprentice relationship kind), Meiling and Sakuya adds -sama to the Scarlet sisters and Patchouli, and Youmu does the same for Yuyuko, etc.)
Sanae has a tendency to refer to everyone with a -san (except for her two goddesses who get -sama) though. Then again she's a schoolgirl from the outside world, so...
